Intake Coordinator
We are looking for an Intake Coordinator to work out of our downtown office.
The Intake Coordinator’s primary role includes
- Provide weekly information (orientation) sessions explaining the services at Champions to potential clients.
- Hold 1:1 appointments to determine service needs requirement of clients and determine if they are ready, willing and able to access Champions services.
- Enter Service Needs Determination information on AEI Data Base (Mobius System), and update client files based on the outcomes of the 1:1 appointments
- Liaise with community agencies via telephone, presentations, and email to ensure cross referral opportunities.
- Have knowledge of all disability types (physical, intellectual, mental health, visual impairments, hearing impairments and addictions) and stay up-to-date on the disability resources in Calgary and surrounding areas.
- Collect and submit information required for client file re-open requests.
- Participate as a key member of the team – supporting policies and overall provision of programs and client services.
- Provide back-up reception coverage.
Qualifications
- Highly motivated and innovative
- Able to manage their time effectively and multi-task
- Have a sound knowledge of disability agencies in Calgary and experience working with a variety of disability groups
- Proficiency in Microsoft Office and experience with databases
- Excellent presentation, communication and interpersonal skills
- Related post-secondary degree and/or relevant experience
- Preference will be given to candidate with experience/degree in Social Work
- Preference will be given to candidates with experience in ASL
